Script Requests

  • Your script request will be processed within 5 working days. For clinician error we do our best to expedite this.
  • This form is for patients who have already seen a doctor or nurse practitioner at Kin Clinic who require a one-off repeat prescription of a medication they are stable on.
  • Changes in dose or type of medication, must be discussed at a review appointment.
  • If you would like to request an out of session script, please complete the below form which is directed to your clinician's secretary — no need to call.
  • Please kindly note that once the request is submitted Doctors and Nurse Practitioners will review the request. Clinicians reserve the right to decline the request if they have concerns about the safety of re-prescribing out of session without reviewing you. In these circumstances no out of session fee script would be charged, and the Kin Clinic Team will contact you to offer a medication review session as soon as possible at the standard rate.
