Adult or Child Psychology Referrals

We ask the GP Referrals to Psychology please kindly include:

  • GP Name, Practice & Provider Number
  • Addressed to: Psychologist at Kin Clinic (you don’t need to specify name of clinician)
  • Reason for referral
  • Current Medication & allergies
  • Past Medical/ Psychiatric History
  • Any Needs considerations: Intellectual Disability, Deafness, visual impairment.
  • Alcohol or Substance use issues known
  • Any Current risk issues
  • History of risk: month and year of most recent suicide attempt or self-harm episode
  • Discharge date of most recent inpatient mental health admission/crises team referral

Mental Health Treatment Plan (MHTP)

Criteria: a diagnosed mental health condition (e.g. anxiety, depression), assessed and confirmed by a GP, psychiatrist, or paediatrician.

What it gives: an initial referral for 6 sessions with a mental health professional (psychologist, or other allied mental health worker), with the option to return to the GP for a review and further sessions up to 10 individual sessions per calendar year. Separately, up to 10 group therapy sessions per calendar year are also available (family/couple therapy doesn’t count as group therapy). The plan itself doesn’t expire, but the session entitlement resets each calendar year.

Eating Disorder Treatment and Management Plan (EDP)

Criteria: a clinical diagnosis of anorexia nervosa (no further criteria needed), or a diagnosis of bulimia nervosa, binge eating disorder, or OSFED plus an EDE-Q score ≥3, evidence of rapid weight loss/frequent bingeing/compensatory behaviour (3+ times a week), and at least 2 of a set of risk indicators (e.g. underweight, medical complications, hospital admission in the past 12 months, inadequate response to prior treatment).

What it gives: up to 40 psychological treatment sessions and 20 dietetic sessions over a 12-month period, plus assessment, treatment planning, and ongoing review.
