Adult Referral Requirements to attend Psychiatry or Mental Health Nurse Practitioner

A GP referral is required to see Psychiatrist or A Nurse Practitioner for safety reasons, so we are aware of your history, and this is also a requirement for Medicare rebates.

  • GP Name, Practice & Provider Number
  • Addressed to: Psychiatrist at Kin Clinic/ Nurse Practitioner at Kin Clinic (you don’t need to specify name of clinician)
  • Reason for referral
  • Medical conditions (psychiatric and physical diagnoses)
  • Current Medication & allergies
  • Past Medical History (including cardiovascular, seizures, liver disease, neurological & any other physical condition; any family history of heart disease)
  • Any Needs considerations: Intellectual Disability, Deafness, visual impairment.
  • Alcohol or Substance use issues known
  • Any Current risk issues
  • History of risk: month and year of most recent suicide attempt or self-harm episode
  • Discharge date of most recent inpatient mental health admission/crises team referral
  • Pregnancy / contraception status in women of reproductive age

For ADHD referrals please ask your GP to additionally include:

  • Weight, Height, BMI, Blood Pressure, Resting Heart Rate.
  • Bloods: FBC, iron/ferritin, TFTs, UEC, LFT, fasting lipids and metabolic profiling, B12/folate

*ONLY for patients with any Cardiovascular risk factors

If there is a personal/family history of heart disease, symptoms like palpitations or exertional fainting, or other cardiovascular risk factors an examination & ECG under a Cardiologist and the Cardiologist to write a letter signing off that it is safe to start stimulants is required.

*ONLY for patients based in Western Australia

age 16+ they must provide an additional Urine Drug screen

Duration: Typically GPs provide 12 months validity of referral. Lasts the 12 months starts from the date of the first specialist appointment, not the date on the GP letter. Or GPs can make an “ongoing referral.” Once issued, it remains valid for the life of the patient-specialist relationship (i.e., as long as the patient continues seeing that specialist for that condition).
